How to Sign-In

1. Locate the QR Code – The QR code will be displayed at the event check-in area.

2. Scan the Code – Use your phone's camera or a QR scanner app to scan the code.

3. Enter Your Information – Enter your first name, last name, and check the box for meeting or event.

4. Confirmation – Once submitted, your attendance is logged, and you receive a confirmation message.

Retrieving Sign-In Information

​There are 4 ways to retrieve the sign-in information:

1.Text Scott @ 317-506-8854 and ask him to send the event information via email. Specify the day of the meeting or event. 

​

2. Boom Form App.– Download the Boom Form App. and type in the Token # CE6858A0 (to be clear, the last character is a number, not a letter). We have two types of forms, so you will see two choices: "NEW MEMBER APPLICATION" and "SIGN-IN." Of course, click the Sign-in. The default is to show the Last 7 days of sign-in activity; however, you may choose other options by clicking the down arrow next to the "Last 7 days."

3. Log into the website – Follow this link: Boom Form | Wix.com

From the top of the page, click "Submissions" and choose "SIGN-IN."

From the right side of the page, click "Export" then click "CSV" to export to Excel. 

Click the blue EXPORT button.

4. Click the link below – The Sign in sheets will be provided in a link below within 24 hours after the sign-in and will be stored here for Board Members access.
